We participate in the following Medicare Advantage Plans:

We advise you to contact your insurance provider before your initial appointment to confirm our participation. Given the extensive variety of individual plan types associated with the companies mentioned, there maybe a few select plans with which we are not in-network. Additionally, we do not participate in most plans offered through the Healthcare.gov Marketplace.
If you do not have insurance, we offer very affordable self-pay rates.
We participate with the Accountable Care Organization (ACO) through the West Florida ACO.
We do not perform services for Workman’s Compensation or for Motor Vehicle Accident injuries.
We do not accept Medicaid as either a primary or secondary form of insurance.
