A few reminders to ensure efficiency:
Please be sure to have Dr. Ehab Michael assigned as your PCP if your insurance requires PCP selection to ensure proper processing of claims and referrals.
Please be aware that if your insurance requires prior authorization for referrals it may take up to 2 weeks to process.
Remember to request refills at least 3 days prior to running out!!

Your First Appointment
Please arrive 30 minutes before your scheduled appointment time to fill out the necessary paperwork. (15 mins if you have already completed your paperwork)
Please bring your new patient forms, picture ID, insurance card, your method of payment, and any prescriptions you are currently taking.
If you are seeing any specialists, please bring their contact information.
